Location: 808 5th Street, SW, Roanoke (On-site, 9am-4pm, M-F)
Job Type: Contract, 30-35 hour/week
Pay rate: $20-23/hour
Overview: We are seeking a compassionate, dedicated, and organized Maternal Health Support Coordinator to join our team. This role is designed to provide increased capacity for Huddle Up Mom’s programs and initiatives. The ideal candidate will be passionate about maternal health, have experience in connecting individuals with community resources, and possess strong communication skills to effectively collaborate with clients and community partners.This role is essential to our Support Shop, which provides mothers with essential baby items, and our Moms Under Pressure blood pressure self-monitoring program for pregnant and postpartum individuals. This role will also provide limited administrative support to daily operations and organizational needs.
Key Responsibilities:

Moms Under Pressure Care Coordination:

Enroll clients into the program and provide ongoing check ins and support throughout one year postpartum.

Collaborate with community partners for enrollment opportunities, such as tabling at health fairs, community events, etc.

Work directly with pregnant and postpartum individuals enrolled in the Moms Under Pressure program to ensure they have knowledge of appropriate community-based services, resources, and health education.

Coordinator will work closely with the Program Director to ensure deliverables of the program are met.

Support Shop Coordination:

Coordinate requests for essential baby items from clients and community partners, and fulfill orders to be picked up from the Haven on 5th.

Maintain up-to-date knowledge of community resources and support services, such as parenting classes, mental health support, lactation consultants, and childcare services. Connect clients with these resources as needed.

Track and maintain day-to-day knowledge of Support Shop inventory and needs and communicate needs to the Director of Support on an ongoing basis.

Administrative Support:

Administrative operational support including, but not limited to: data entry and management, document preparation and management, and general office management

Fundraising, event coordination, and marketing and outreach support, as needed

Program assistance and volunteer coordination, as needed

Financial support including expense tracking and donation management, as needed

Estimated percentage of time: 10%

Qualifications:
-
Education: Bachelor's degree in public health, nursing, social work, or a related field. Certification or training in maternal health, case management, or a similar area is a plus. Relevant work experience may be considered in place of degree requirements.

Experience: At least 2-3 years of experience in non-profit or healthcare program delivery, maternal health, case management, or patient coordination. Experience working with diverse populations is preferred.

Strong communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.

Excellent organizational and time-management abilities with minimal supervision.

Ability to handle sensitive issues with empathy and professionalism.

Knowledge of maternal health issues, including prenatal and postnatal care and mental health are a plus.

Work Environment:

This position may require occasional travel to community centers or events.

May involve some evening or weekend work based on client needs and program schedules.

Must be able to lift at least 15-20 pounds.
